Introduction 

This Privacy Policy explains how I collect, use, and protect your personal health information when you use my nutrition services, including through online booking, virtual appointments, and communications. I follow the requirements of Ontario’s Personal Health Information Protection Act (PHIPA). 

What Information I Collect 

When you book an appointment or use my services, I may collect:

• Your name, email, phone number, and address.
• Relevant health information that you provide (e.g., medical history, medications, nutrition concerns).
• Appointment history and payment information.

How Your Information Is Used 

Your information is used to:

• Provide safe, evidence-based nutrition care.
• Schedule appointments and manage billing.
• Communicate with you about your care.
• Maintain records as required by law and the College of Dietitians of Ontario.

How Your Information Is Stored 

• Records are stored electronically in secure systems (including Square and encrypted file storage).
• Some services may store data on servers outside of Canada (e.g., in the United States).
• All reasonable steps are taken to protect your privacy and data.

Retention of Records 

• Records are kept for at least 10 years after your last visit, or 10 years past your 18th birthday for minors, in accordance with Ontario regulations. 

Access to Your Records 

You have the right to:

• Request a copy of your records.
• Ask for corrections to your records.
• Withdraw consent for certain uses, except where disclosure is required by law.

Sharing of Information

I do not share your personal health information with third parties without your consent, except when required by law (e.g., risk of harm, court order, public health reporting). 

Virtual Care Disclaimer 

Virtual appointments are conducted by secure video or phone. While all reasonable measures are taken to protect your privacy, electronic communication may carry some risk of interception. By booking, you consent to receiving care through these methods.
